Safe Work Permits
Issue, monitor, and terminate Streamline Safe Work Permits for higher-risk or permitted work. Live permits stay editable until they're closed; closed permits become a permanent record.

Up to 9 crew members. The first row is auto-filled from the Person in Charge above.
Add a review entry whenever the JSA is reviewed during the job — periodic check-in, scope/condition change (requires new JSA), or after an incident.
Welding / Cutting / Grinding / Non-intrinsically safe equipment. Complete items 1-6 prior to beginning hot work.
★ Fire Watch person shall have no other duties while hot work is in progress. Per regulation, FW shall maintain watch at least 30 min (Permian) / 1 hour (Colorado) after completion.
Safe limits: O₂ 19.5–23.5% · LEL <10% · H₂S <10 ppm · CO <35 ppm. Add a new row each time a reading is taken.
Trenching / Excavating / Hydrovac / Augoring / Boring. Complete items 1-8 prior to beginning ground disturbance. If excavation depth is 4 ft or greater, complete the Excavation Entry checklist that appears below.
Record each inspection performed during the work. Excavations must be inspected daily by an Excavation Competent Person.
Complete items 1-10 prior to entering an excavation 4 ft deep or greater.
Pick the entry path that applies. Reclassification is for spaces that meet all 8 conditions to be entered as non-permit. If any condition fails, use the Permit Required Confined Space Entry (PRCSE) path.
Complete items 1-8 prior to entry. All 8 items must be Y to reclassify the space as non-permit. If any item is N, the PRCSE path is required.
Complete items 1-9 prior to permit-required confined space entry.
Entrants shall review atmosphere test results prior to each entry. Each entry/exit must be logged.
Identify the equipment to be isolated, the energy sources, the isolation devices, and the authorized employees who will apply locks & tags. Each isolation device must be verified by Lock / Tag / Try.
List items in order of isolation / operation. For each device, mark its position (Normal / Isolated) and verify Lock / Tag / Try.
Complete items 1-7 prior to restoring equipment. Check each isolation device when returned to normal operating position (use the "Restored" column above).
List each authorized employee who applies a lock & tag. Mark the first row as Primary Authorized for Group/Full lockouts. Record date/time when locks & tags are attached and again when removed.
Approval is signed by the Streamline Representative when the permit is issued. Termination is filled in when work is complete (or the permit is otherwise ended).
Terminate Permit
Closing a permit is permanent — once terminated, the permit moves to the Closed list and can no longer be edited.
Roll Off Swap
Safety inspection record for roll-off bin swaps. Complete this form before, during, and after each swap.
